AnMed provides credentialing services as part of our billing service contract, which is especially important for new practices. However, we also provide these services for a fee for providers who just need help obtaining new provider numbers.

The following is a list of fees for each application:

• Corporate Application for Medicare, 3 part – $300.00
  
• Single Application for Medicare (Sole Proprietor App) – $150.00
  
• State of Illinois, Health Care Professional Credentialing and Business Data Gathering Form – $100.00 initial
  (Multi-use form) $25/copy
  
• Wellmark Blue Cross Blue Shield Application – $100.00
  
• Illinois Department of Public Aid Application – $50.00
  
• Iowa Medicaid/ACS Application – $50.00
  
• Illinois Blue Cross Blue Shield Application – $50.00
  
• Yearly re-credentialing existing provider – $25.00
  
• Correction/addition to existing provider file – $25.00
